Social Services
Our social services team provides comprehensive support through assessment and care coordination, counseling and emotional support, and connecting participants with essential community resources and advocacy.
Assessment & Care Coordination
Our social workers conduct thorough assessments to understand each participant’s unique needs and circumstances. They collaborate with our interdisciplinary team to develop and update individualized care plans, ensuring services are tailored to support health, independence, and quality of life.
Counseling & Support
We provide individual-centered, one-on-one counseling to help participants manage emotional challenges, adjust to changes in health, and strengthen coping skills. Our social workers also offer education, guidance, and ongoing support to both participants and their caregivers.
Resource Connection & Advocacy
Our team connects participants and families to vital community resources such as housing, transportation, and in-home support services. We advocate for participants’ rights and needs, and respond to urgent situations to ensure safety, stability, and access to appropriate services.
