Eligibility

If you have a valid California Medi-Cal card, are at least 18 years old, and have a current doctor’s order, you can receive our services at no cost. We also offer the option to pay out of pocket.

 

如果您持有有效的加州白卡（Medi-Cal）、年滿18歲，並持有醫生的有效證明，您即可免費享受我們的服務。我們也提供自費服務的選擇。

Application Process

Please download the Physician’s Health Assessment Form and have your physician complete it in full, including a Chest X-ray report from the hospital dated within the past year. Once you have both documents, please contact us, and our staff will assist you with initiating your application.

Please bring the following items when visiting Mikkon ADHC

  • California ID

  • Social Security Card

  • Medi-Cal Card

  • Medicare Card (optional)

  • Managed Care Insurance Card

  • PCP’s H&P Form, plus TB clearance or Chest X-ray Report

  • Emergency Contact (caretaker’s name and contact information)
